Media Release Form

By signing the form below, you are granting Spicewood Vet Clinic permission to use portions of sound, video clips, or photographs of your pet, who has been or is currently a patient at the clinic, for promotional purposes. Additionally, you are giving us permission to use your pet's name.

List All Pets Covered By This Release*

While your pet is receiving treatment at Spicewood Vet Clinic, we may record audio and/or video of approved pets.

By giving my permission, I authorize Spicewood Vet Clinic to use the recorded sound and/or visuals, as well as the name(s) of my pet(s), for the following purposes:
Media activities currently planned or in the future, which will help Spicewood Vet Clinic with resource needs

The promotion and advertisement of Spicewood Vet Clinic

Any other commercial purposes

I understand that by granting this permission, Spicewood Vet Clinic has the right to use the recorded sound and/or visuals in any way it sees fit for the above-stated purposes. Spicewood Vet Clinic may reproduce the sound and/or visuals in whole or in part, and distribute them using any medium, including the internet, CD-ROM, or other multimedia platforms.

I acknowledge that Spicewood Vet Clinic will not provide compensation to me for granting this permission or for any use of the sound/visual materials.
